  • Accident: 14509202 - Killed When Struck By Backing Trailer

    Employee #1 was at a job site where a tractor trailer had just dropped off a connection box. The driver was backing up in preparation for pulling onto the roadway when Employee #1 was struck by the trailer. He was killed.

    Accident: 14225841 - Killed When Struck By Backing Truck

    Employee #1, a contract truck driver, had just delivered a load to the Best Buy Company's distribution center and was walking south in the parking lot to return to his tractor trailer. Another contract driver entered Best Buy, drove past Employee #1's vehicle, and began to slowly back up in a straight line. When the driver turned left to align his tractor with the trailer, he saw Employee #1 lying in the parking lot. He had been struck and killed by the backing truck.

    Accident: 896464 - Killed When Caught Between Loader And Dump Truck

    Employee #1 was struck by a Volvo front end loader as it was backing up. He was killed when he became pinned between the loader and a dump truck.

    Accident: 100600527 - Killed When Struck By Backing Garbage Truck

    Employee #1 was helping to unload furniture at the rear of a private residence. He was standing directly behind his truck and had his back to the entrance of a dead-end alley. A garbage truck decided to back into the alley because it could not turn around without going out into a muddy field. The truck did not have a back-up warning device, and vision was obscured to the rear for the driver. A "swamper," or helper, was riding on the back of the garbage truck and could see what was going to happen. He waved his arms, yelled at the driver, and did what he could to get the driver's attention, but to no avail. There was a button at his station that he was supposed to use to notify the driver to slow down or to stop; however, the helper was normally a truck driver, who was being used as a swamper because the company was short on people to load garbage. He knew about the button, but froze when he saw that the trucks were about to collide. The trucks collided, and Employee #1 became pinned between the two; one of his legs was amputated and his diaphragm and lower body were crushed. Employee #1 was immediately evacuated to the local hospital, where he died 4 hours later.

    Accident: 883900 - Killed When Run Over By Asphalt Truck

    At approximately 4:15 p.m. on October 1, 1998, Employee #1 was engaged in taking road measurements when he was run over by an asphalt dump truck that was backing up. Employee #1 was killed.

    Accident: 658278 - Killed When Struck By Backing Tractor Trailer

    Employee #1, a groundsman for a road milling machine, was walking toward the milling machine, which was in position to start another cut in the road. At the same time, a 14-wheel dump truck and a tractor trailer were backing up to the milling machine. Employee #1 walked across the path of the tractor truck and was struck by the right rear wheels of the dump trailer. He was killed. The tractor trailer did not have a back-up alarm, and there was no observers to signal the drivers that it was safe to operate their vehicles in reverse.

    Accident: 999110 - Killed By Backing Bulldozer

    Employee #1 and coworkers were constructing a driveway. One coworker was operating a bulldozer while Employee #1 was behind the bulldozer measuring the grade of the earth with a laser. The bulldozer backed over him. He was crushed and killed.
