  • Accident: 200621688 - Run Over By Forklift

    A carnival worker was climbing into a metal basket while it was being moved by a boom attached to the front end of an R80 Caterpillar forklift truck. The employee lost balance, and his foot got caught under the left front wheel of the truck. He was pulled under the forklift, and both wheels ran over him before the driver could stop it. The carnival worker died of his injuries.

    Accident: 170572838 - Fall - Wire Rope Support Failure

    An employee was operating a Climb-N-Dangle rock-climbing wall at an amusement park. The wire rope by which he was suspended broke, allowing him to fall 4.6 meters to the ground. He fractured his right wrist in the fall.

    Accident: 200270833 - Crushed While Greasing Amusement Ride

    Three employees were greasing a carnival ride. Two of them were under the ride. The third employee was standing on a platform above the ride. One of the employees under the ride (employee #1) instructed the worker next to him to hold the end of the grease gun while the first employee pumped the gun. Employee #1 instructed the employee on the platform to begin turning the upper rim of the ride, which sat at a 15-degree angle during operation. As the employee on the platform began to turn the rim, the heaviest part of the ride rotated down hill. Its momentum caused the ride to continue turning. The second employee under the ride dove to the ground out of the way of the rotating supporting structure. Unfortunately, employee #1 was crushed between the ride's rotating turret and the stationary trailer frame. He died of his injuries.

    Accident: 200782084 - Electric Shock - Contact With Energized Parts

    A 21-year-old maintenance worker for an amusement park was found floating face down in a shallow pool of water following his attempt to change a lighting fixture. He had been electrocuted.

    Accident: 201793288 - Injured In Fall From Amusement Ride

    A church had hired a recreational services company to set up a fair with some amusement rides. A member of the public, a 16-year-old girl, was riding in a swing ride called a "Psycho Swing." The church had provided two volunteers to operate the swing. The swing was one-person, manually operated rigid swing, which can perform 360-degree revolutions. It was equipped with a braking device and had a strap on the left side. The rider, wearing a harness and holding onto the two vertical supports, stands on a platform that revolves around an axle. One of the volunteers hooked the swing's snaphooks to the harness on the girl and clamped her feet to the platform. The girl did a loop, went forward and backward, then slipped off, flipping and landing chest first, face down. She was still wearing the harness when she landed. She was hospitalized for her injuries. (No employees were injured in the accident.)

    Accident: 201062973 - Entangled In Net With Dolphin

    Seventeen employees were performing a dolphin capture exercise in a dolphin stadium at a marine amusement park. They were working in the holding pool in the stadium. This was the fourth time this particular operation had been done at the pool, each time with a different pair of dolphins. The two dolphins to be captured were being guided by a nylon net toward a shallow area of the pool (called a "slide out") for holding and transport. One of the employees (employee #1) was free diving in the pool on the opposite side of the net from the dolphins. One of the dolphins had already been captured, but the second young dolphin was not familiar with the net and was swimming in the deeper part of the pool as the net was being brought around. Other employees both in and out of the water were responsible for pulling the net toward the slide out and keeping the net taut so that the dolphin could not escape. The dolphin swam into the net toward employee #1 near the northeast corner of the pool, about a meter from the slide out, in about 3.7 meters of water. Employee #1 grabbed the dolphin through the net to subdue the animal and bring it to the surface. The dolphin was calm for a moment and then began to spin, entangling both the dolphin and the employee in the net and pulling them deeper. The employee sustained three fractures and torn ligaments in her right arm, which was entangled in the net. Other employees brought her to the surface while she was still conscious. The injured employee was hospitalized.

    Accident: 201153277 - Hand Caught In Shredder

    An employee was working at a prize counter counting tickets at an amusement park. As she was inserting tickets into an Allegheny Paper Shredder, her right hand contacted the blade, amputating her right index, middle, and ring fingers. She was hospitalized for seven days for her injuries.
