Accident: 170205983 - Injured In Fall From Ledge
At approximately 11:15 a.m. on April 7, 2000, Employee #1, a supervisor of a specialty department at a whole foods market, was on his break in the smoking area. He and a coworker went to the back of the store, and he hoisted himself onto the 42 in. high parapet to sit on the ledge. He underestimated the hazard and fell backward. Despite a previous shoulder injury, the coworker managed to hold onto him for few minutes while yelling for help. No one responded and shortly thereafter, the coworker lost his grip. Employee #1 fell about 19 ft, landing on his feet. He fractured both his ankles and was transported to San Francisco General Hospital for treatment.

Between 7:00 and 7:20 a.m., Employee #1, a cafeteria cook for a medium-sized public elementary school, was carrying a box of frozen Tater Tots from the freezer box to the kitchen. As she left the freezer, her foot slipped out from under her on the threshold. She fell backward, fracturing her left ankle. Employee #1 was taken to Rideout Hospital in Marysville, CA, where she was treated for approximately five days.

Accident: 170205876 - Fractures Ankle In Fall While Descending Stairs
At approximately 1:00 p.m. on January 18, 2000, Employee #1, a mail sorter for Community Vocational Enterprises, was leaving work . She began a search for her identification card while walking down the stairs. She was preoccupied and when she reached the third of four steps, she tripped and fell forward. Employee #1 fractured her left ankle and was transported to the hospital. She underwent surgery that same day, and was released on January 21. No accident-related violations were found in connection with this incident.

At approximately 1:30 p.m. on August 19, 1999, Employee #1, of Vincent B. Zaninovich & Sons, was unloading empty styrofoam grape boxes from a swamping truck when the driver suddenly moved the vehicle forward. Employee #1 lost his balance and fell off, suffering a serious injury to his left ankle. He was admitted to Delano Regional Medical Center in Delano, CA.

Accident: 170205850 - Injured When Struck By Falling Steel Column
At approximately 11:45 a.m. on August 5, 1999, Employee #1, an iron worker, was on the third level of a building under construction. A tubular steel column was being hoisted to the fourth level, where several coworkers were ready to perform the connecting work. The top of the column was cleared and positioned to its connecting point. The bottom of the column, however, was still being raised when it became caught on a beam. The spotter had called to the tower crane operator to stop when the choker parted and the column fell approximately 10 ft to the third level. It struck Employee #1, pinning him to the deck. He suffered ankle and head injuries, and was transported to San Francisco General Hospital.
