    Employee #1, a sheet metal installer, was patching holes around duct work in the attic of a building that was being remodeled. He stepped through the ceiling and fell 25 ft. Employee #1 was killed.

    Accident: 170574032 - Electric Shock - Possible Ground Fault In Electrical Box

    An employee was working in the attic of a house, installing new air conditioning equipment. He contacted an energized object in the old attic wiring (possibly the metal case of a junction box) while another part of his body was touching a grounded metal pipe. The employee was electrocuted.

    Accident: 170570345 - Injured In Fall When Acoustic Panels Give Way

    Employee #1 was standing fire watch in the attic of the Statehouse when he stepped off a metal deck and onto acoustic panels. The panels gave way, and he fell approximately 70 feet. The sides and edge of the deck were not protected.

    Accident: 200030112 - One Killed, One Injured In Roof Collapse

    Employees #1 and #2 were working in the attic of a three-story building that had been condemned due to fire. The work required the removal of roofing material and roof trusses, which was done from inside the attic. They had removed about two-thirds of the roof when the remainder collapsed on top of them. Employee #1 was killed and Employee #2 sustained injuries that required hospitalization.

    Accident: 170630719 - Injured In Fall Through False Ceiling

    Employee #1 was servicing an electrical motor in the attic of a building when he lost his balance and fell 15 ft through a false ceiling. He landed on a concrete floor, sustaining multiple injuries and lacerations.
