At approximately 5:15 p.m. on October 25, 2000, Employee #1, a recreational vehicle repair technician, was installing a leveling jack on a recreational vehicle when he sustained a serious groin injury involving a ruptured right testicle. Employee #1 drove himself to his home in Riverside, where paramedics were summoned. He was transported to Kaiser Hospital, where he was treated and hospitalized for five days.

Accident: 116211962 - Crushed And Killed Under Truck
Employee #1 was working on the starter of a Freightliner truck cab, which was supported by a wooden ramp under the front wheel on the driver's side. The truck wheels were not chocked, and the truck did not have brakes. While Employee #1 was underneath the truck, it rolled forward and pinned him in place. He was crushed and killed.

At approximately 7:45 a.m. on November 18, 1999, Employees #1 and #2, of Amrep, Inc., were working on the body of an elevated refuse truck when the roll-off bin came down on top of them. Both sustained traumatic head injuries. Employee #1 was pronounced dead at the scene; Employee #2 was hospitalized.

Accident: 170193221 - Injured When Struck By Brake Drum And Wheel Hub
Employee #1 was working at an equipment parking and shop area owned and operated by Wyroc, Inc., a surface sand and gravel mining operation. He was helping a coworker remove a brake drum and hub from the wheel of a Ford 950 truck when he slipped on the gravel surface. The brake drum and hub fell on Employee #1's left leg. He sustained injuries, but was not hospitalized.

Accident: 18181560 - Dies Of Burns Sustained In Gasoline Fire
At approximately 2:00 p.m. on March 2, 1998, Employee #1 removed a fuel tank from a 1988 Chevrolet Astro minivan to replace a bad fuel pump. The fuel tank, which was approximately three-quarters full, was balanced on a portable hydraulic jack. It tipped over and gasoline spilled onto the shop floor. The gasoline vapors reached a source of ignition, which apparently was the pilot light/burners on the gas furnace, which were within 8 ft of where the fuel tank had been removed. A fire erupted and Employee #1 sustained burns, from which he died.

Accident: 170564967 - Dies Of Asphyxia When Pinned By Truck Cab
On February 19, 1998, Employee #1 was replacing the torsion bars for the cab tilt mechanism on a 1993 Peterbilt cab-over truck. At approximately 10:00 p.m., he was found bent over in the right front wheel well, pinned by the closed truck cab's weight. Employee #1 died of asphyxia from external thoracic compression. He apparently did not use the adjustable safety braces supplied by his employer to support the raised cab.

Accident: 201630068 - Injured When Pinned By Automobile
At approximately 11:30 p.m. on September 1, 1997, Employee #1, a gas station attendant, was directing an automobile into a garage stall when the driver of the vehicle accelerated and pinned him against a workbench. Employee #1 suffered multiple fractures to his legs and hands, and lacerations to his lips and nose.
