Accident: 170611925 - Injured When Impaled By Flying Piece Of Wood

Employee #1 was using a Mattison rip saw, model #202, to straight line a 10 ft by 4 in. by 2 3/4 in. maple board. He adjusted the saw to cut a clean edge on the board and then put the board into the self-feeding saw. The drop-off from the cut became lodged under the stock. After it had been cut, he reached for the board to pull it back. The 1/8 in. by 2 3/4 in. drop-off caught the blade of the rip saw and kicked back, striking and impaling Employee #1. He sustained injuries that required hospitalization.

Accident: 896589 - Injured When Neck Impaled On Gear Shift

Employee #1 was using an over-the-road tractor truck to pull a house to a new location on a trailer-type assembly. The chimney, which had been braced and supported, became dislodged from the house and fell onto the cab of the truck. Employee #1's neck was impaled on the gear shift, and he was hospitalized for treatment of his injuries.

Accident: 638262 - Injured In Fall Onto Rebar

Employee #1 was laying concrete blocks from a scaffold platform when the platform planks became dislodged and fell. Employee #1 fell onto an unguarded and unprotected metal reinforcing rod 11 ft below the platform. The reinforcing rod pierced his scrotum from underneath, for which he was hospitalized.

Accident: 170724629 - Fell From Catwalk And Was Impaled On Angle Iron

A heavy equipment mechanic was assisting a crew that was removing a temporary steel support structure after the repair of a water turbine at the bottom of the turbine pit. The mechanic was on a catwalk. The catwalk was not protected by guardrails, and the employee was not using personal fall protection equipment. He accidentally stepped back off catwalk and fell, landing on a vertical piece of angle iron lying on the concrete floor. He was impaled on the upper part of his left thigh and suffered a shattered hip socket. He was hospitalized for his injuries.
