    Accident: 119948008 - Injured While Servicing Recreational Vehicle

    At approximately 5:15 p.m. on October 25, 2000, Employee #1, a recreational vehicle repair technician, was installing a leveling jack on a recreational vehicle when he sustained a serious groin injury involving a ruptured right testicle. Employee #1 drove himself to his home in Riverside, where paramedics were summoned. He was transported to Kaiser Hospital, where he was treated and hospitalized for five days.

    Accident: 170366280 - Head Crushed By Falling Forklift

    Employee #1 was working under a forklift. He had a jack behind the left front wheel and a block behind the left drive wheel. The jack kicked out from under the forklift , which then fell on his head. Employee #1 was killed.

    Accident: 100810456 - Killed When Struck By Pump Jack Counterweights

    Employee #1 was on a pump jack scaffold, preparing to repair a scrapper bar when the unit activated. He was struck by the scaffold counterweights and was killed. The pump jack had not been turned off.

    Accident: 515932 - Killed When Crushed Between Modular Buildings

    Employee #1, age 20, was installing modular building units, which he raised and lowered with a Simplex jack. He was walking between two of the units, one of which was raised, when it slipped and fell toward the other unit. Employee #1 was crushed between the modular buildings; he died of asphyxia.

    Accident: 655993 - Hand Crushed By Falling Radio Tower

    Employee #1 was sweeping debris from under a 300 ft high, 14,000 lb radio antenna that had been raised by two 10-ton hydraulic jacks to facilitate replacement of a damaged base insulator. One of the jacks twisted and the tower fell, crushing Employee #1's right hand.
