Accident: 784728 - Killed When Crushed In Transfer Conveyor

At approximately 9:45 a.m. on July 21, 2000, Employee #1 was operating a transfer conveyor that jogged up and down to move lumber. A scrap of wood became jammed in the machine and he reached through the framework of the conveyor to clear it and to activate the machine's switch. The conveyor raised up and caught the midsection of Employee #1's body. He sustained crushing injuries and died the following day during surgery.

Employee #1, the lead person on the shift, was reaching into a moving conveyor belt to clear a jam when his hand was pulled into the belt. He sustained injuries that required hospitalization.

Accident: 119948487 - Fingertip Amputated In Meat Grinder

At approximately 5:40 a.m. on June 7, 2000, Employee #1 was trying to clean or clear a product jam from the discharge port of a Butcher Boy meat grinder. In the process, he amputated a portion of the first digit of his left index finger. Employee #1 was transported by private automobile to Kaiser Permanente Hospital, where he was treated and released.

Accident: 113549968 - Fingers Injured In Press

Employee #1 was dislodging a piece of material from a press when he and a coworker accidentally activated the press. His index finger and thumb became caught, and he sustained serious injuries.
