    Accident: 200782084 - Electric Shock - Contact With Energized Parts

    A 21-year-old maintenance worker for an amusement park was found floating face down in a shallow pool of water following his attempt to change a lighting fixture. He had been electrocuted.

    Accident: 120126479 - Injures Back In Fall

    At approximately 8:15 p.m. on March 11, 2000, Employee #1, a theatrical rigger, was removing a theatrical lighting arrangement at the Ontario Convention Center in Ontario, CA. The elevating work platform he was using tipped over and he fell 14 ft 6 in. Employee #1 sustained serious injuries to his upper back and was transported by paramedics to Loma Linda University Medical Center, where he was hospitalized for ten days.

    Accident: 14242416 - Electric Shock - Direct Contact With Energized Conductor

    An electrician was working on a 277-volt lighting circuit without deenergizing it. He contacted an energized conductor as he was making a connection in a switch box and was electrocuted.

    Accident: 126008895 - Killed When Run Over By Harvesting Tractor

    At 10:00 p.m. on June 3, 1999, Employee #1, an experienced and trained produce harvester who had been harvesting corn in a field with a crew of 28 coworkers, was discovered to be missing. It was assumed that he had left the harvesting crew early and had departed the work site. Five days later, Employee #1 was found dead in the same field in which he had last been seen. He appeared to have been hit or run over by harvesting equipment. The tractors in use in the field were International 5130s. Employee #1 died of internal injuries, including at least three fractured ribs. Causal factors could not be determined due to the lack of witnesses and evidence at the scene. However, working at night and low visibility due to dust and darkness may have contributed.

    Accident: 126008366 - Killed In Fall Into Storage Pit

    On January 12, 1999, Employee #1 was engaged in cleaning in a building that was utilized by two aircraft engine rebuilding and repair companies. He apparently entered a darkened room or area and fell 8 ft onto the concrete floor of an unguarded storage pit. Employee #1 suffered major head trauma and was killed. He had fallen either from the floor edge of the storage pit or from a stepladder that was sometimes used to access the pit. Employee #1's body was found by a security guard in a part of the facility that belonged to another company in the building. Apparently, Employee #1 was not authorized to enter this area of the facility. Factors contributing to this accident were Employee #1's unauthorized entry into this darkened area and his misjudgment of a hazardous situation. He was not an employee of the owner of the property.
