    Accident: 170040737 - Dies After Suffering Heart Attack

    On January 24, 2000, Employee #1 was sitting in the cab of a log truck waiting for his turn to be loaded when he suffered a fatal heart attack and collapsed. Employee #1 was pronounced dead as a result of the heart attack.

    Accident: 170040646 - Killed When Struck By Falling Logs

    Employee #1 was a log truck driver for a thinning crew, moving logs between two mechanical logging yarder landings that were approximately 1/4 mile apart. He moved a loader so he could back his truck into position to load it. After reversing up to the loader, he got out and began to remove a wrapper chain binder from the logs. As he did so, two logs fell on him from the driver's side, pinning him to the ground. Employee #1 suffered a head injury and was killed.

    Accident: 170040745 - Killed When Log Truck Overturns

    Employee #1 was driving a log truck northbound on a highway when the truck left the road and went into a ditch on the southbound side of the highway. The truck traveled along the ditch until it overturned when Employee #1 tried to maneuver it back onto the road. The truck came to rest on its roof in the southbound lane of the highway. Employee #1 suffered multiple injuries and was pronounced dead at the scene.

    Accident: 170040901 - Killed When Crushed In Log Truck Cab

    Employee #1 was driving a log truck when, for some reason, he failed to stop and rear-ended another log truck. The shifting load of logs sheared off the cab guard anchors and crushed the top of the cab. Employee #1 became caught between the steering column and the cab roof, and was killed.

    Accident: 170132427 - Log Truck Driver Killed When Truck Goes Off Cliff

    At approximately 8:15 a.m. on July 1, 1998, Employee #1 was returning to Anatone, Washington, in an empty log truck. As he proceeded down a steep grade on Highway 82 at Milepost 31, he lost control of the truck at a 90 degree left curve. He crashed through the guardrail and tumbled approximately 1,500 ft to the valley floor. Employee #1 was thrown from the vehicle and died on impact.
